PATIENT NOTIFICATION OF INSURANCE
PARTICIPATING STATUS TERMINATION 

Dear Patient:

I wanted you to hear from me personally that after careful consideration, I have decided to terminate my 20 years of in-network participation with the DenteMax Dental PPO fee schedule.

It has become apparent over the past year that I can no longer continue to provide the personal care and caliber of treatment that you need and deserve under the existing arrangement with DenteMax. Since I cannot ethically sacrifice my patients’ care, the only solution is to withdraw from DenteMax’s network, effective January 1, 2024.

I hope you will remain a patient with our practice. I believe the relationship of trust and mutual respect that we have developed is important. We will continue to submit claims to your individual insurance on your behalf. Your insurance will now pay us as an out-of-network provider, and your out-of-pocket expenses may increase slightly depending on your plan.

DenteMax is not an insurance company, this is a leased fee schedule that many insurance plans use to determine their fees when processing claims.

We ask that ALL patients regardless of your insurance company, call your individual insurance plans to ask if they pay based on this fee schedule. If so, please know your claims will now process as out-of-network.
